Jamio openwork specifically allows the modeling of a generic diagnostic-therapeutic-care pathway ( PDTA ) to manage chronic conditions according to the most advanced best practices, specifying activities, actors involved, defining therapies, identifying potential risks, managing significant and/or adverse events, etc. and to follow up on its progress, integrating diagnostic and laboratory information using international standards ( HL7/CDA2 ) and focusing attention on the patient ( patient empowerment ), who remains at the center of the process also thanks to the use of mobile and cloud-based technologies .
Prof. Dello Strologo, is using a solution designed to support the integrated care pathway for children who have received a kidney transplant. This post-transplant follow-up is governed by a strict, specialized clinical protocol for each patient. All instrumental tests are included, and mechanisms have been established to manage alerts, home events, and reminders for upcoming tests. Bambino Gesù Children's Hospital (Rome) also won the prestigious "WfMC Global Awards for Excellence in Case Management – 2015."
The Clinical Trail Management Framework and eCRF (Electronic Case Report Form) allows Principal Investigators to model clinical pathways online through the abstract and linear definition of activities along organizational, temporal, and geospatial dimensions. The solution is published in public cloud mode and is customized to meet the needs of Principal Investigators , Neurologists, Bioengineers, and Caregivers of Parkinson's patients at IRCCS Ca'Granda and other centers involved in clinical trials, while respecting information visibility and task assignments. Patient enrollment in a clinical pathway generates a calendar of activities that schedules and proposes, depending on the role, diaries, clinical assessments, questionnaires, clinical scales, drug administration activities, device setup and activation/deactivation activities, with the option of receiving notifications and reminders via various communication channels (email, internal messages, etc.).
Openwork led the research project. It developed an integrated ecosystem of practices, processes, and tools supported by ICT technologies for the integrated and patient-centered management of clinical care pathways. The proposed solution's key features are flexibility (including modularity and scalability) and context-awareness, making it adaptable to all chronic disease contexts where a structured diagnostic and therapeutic pathway is available and integration with biomedical or lifestyle measurement systems is possible. The ecosystem, called SmartHealthPlatform, aims to: 1) Enable the definition, execution, and monitoring of clinical pathways; 2) Include in clinical pathways the detection and analysis of patient biosignals, therapies and diagnoses, assessments, and tests, so as to holistically interpret their condition at any given moment; 3) Provide ongoing patient monitoring , enabling the prevention of adverse events or the detection of collective changes in vital and environmental parameters.
Jamio openwork platform was acquired in "private cloud" mode by the IT management of ASL BAT and a Clinical Case Management System (CCMS) . Within the CCMS, it is possible to "model" the PCA, orchestrate information from external healthcare information systems useful for clinical decisions, activate the involved operators and patients enrolled in the relevant PCA. Typically, the solution supports: modeling of treatment protocols, support for clinical decision-making, monitoring, control and analysis of activities, support for planning/execution of activities and finally optimization of activities within the healthcare facility.
The solution implements: 1) Incident reporting management processes based on territorial scope, event severity, organizational roles, and automatic (bottom-up) and manual (top-down) task assignment; 2) Support for the analysis of causes and contributing factors to the occurrence of reports, and for the planning and verification of risk reduction actions; 3) Incident, Event, Cause and Factor Analysis forms as printable attachments, and automatic communication of reported incidents to the appropriate roles and claims to the Legal Affairs Office; 4) Digital generation and transmission of traces ; 5) Integration of system identity management repositories in SSO mode and document systems in the Sardinia region; 6) Litigation and claims management processes and support for the entire process, from compensation requests to settlement of claims; 7) Statistics and reports, both medical (analysis of claims by facility, department, shift, effectiveness of actions, etc.) and administrative (case management times, workloads of the various offices, etc.).
The Center has been recognized as a Regional Center for Neurodegenerative Diseases , including Amyotrophic Lateral Sclerosis (ALS) and other Rare Neuromuscular and Neurodegenerative Diseases, providing similar care. For this reason, the Center can issue Rare Disease Certification and develop Treatment Plans for the conditions it serves. The SaaS software solution on Paas Jamio can simultaneously manage the data flow of patients admitted via "CUP" or "External Reporting" through a multi-level diagnostic pathway.
Within the broader scope of clinical pathway management support systems (PDTA), the project aimed to develop, on the Jamio openwork PaaS , a software solution for performing patient assessment activities, including the creation of questionnaires and self-assessment scales, data collection, consultation, and extraction. The solution is designed in compliance with the privacy and security set by current legislation. Specifically, it guarantees controlled access to data based on user profiles, pseudonymization of sensitive data, and the adoption of GDPR-mandated measures for data encryption and backup. The solution enables the complete dematerialization of the assessment process , eliminating the need to copy data from paper questionnaires to a data storage device. It eliminates the use of IT tools such as spreadsheets to manage data, thus reducing the operational and security risks associated with such management. Specifically, the solution is being used to conduct an epidemiological study conducted by the scientific department on a sample of the population of Castellana Grotte aged over 65. The study is multi-year: a sample of approximately 2,500 people over 65 are called upon over time to undergo multidisciplinary visits and the evolution of their frailty over time is tracked . The study began in 2012, and over time, the data has been collected and archived on spreadsheets.
